Client Forms

I have 2 forms for all of my new clients:

1. Client Consent Form – All NEW clients must complete this form prior to a session. Print it, complete both sides and bring it to your first appointment. If you forget, I also have hard copies available at my office as well.
2. Intake Form – This is an intake form for NEW clients. This form is available to complete online when you book your appointment (it is private and secure). If you’d prefer to print and complete manually you can do so here.
